A one time, non-refundable Community Fee of $2300.00 will be payable when the resident agreement is signed and returned. This fee covers all of the following services:

• Performing the needs assessment to insure the most appropriate level of care by communicating with doctors, healthcare providers, social service agencies and the resident’s family.
• Processing all administrative documentation including health forms, pertinent personal information, medical reports and billing procedures.
• Prepare the residency agreement and all other pertinent documents i.e. current medication requirements and evaluations, which would be immediately accessible in a time of emergency.

Every resident at The Chestnut Cottage at The Elms is entitled to receive our “essential” level of care which includes the following at no additional cost:

• Assistance with bathing up to once a day. Assistance may be cueing or limited physical assistance. This is a service that would occur on a scheduled basis.
• Oversight and limited assistance with other personal hygiene issues (i.e. brushing teeth, shaving, dressing) that can safely be provided by one person.
• Assistance with toileting activities, including occasional bladder incontinence, which may include establishing a scheduled toileting program and/or cueing.
• Medication administration
• Monitoring and supervising the resident’s location and condition including day, evening and night checks.
• A Nursing overview. This includes observations, assessments, development of a care plan, evaluation of that care plan, working with the resident’s physician and hands on care i.e. eye drops and dressing changes. Cueing or limited assistance with traveling to and from the dining area and activities. The resident must be independently mobile (canes, walkers and wheelchairs are acceptable).
• Mealtime oversight to provide encouragement with meals.
